The current registration form is available to download. To access the registration form you must follow the following procedure:

1) Sign in under "Parent Login". If you are not a registered user, please click on "Create an Account"

2) Click on "Downloads" under the Important Information Side Menu   

3) Click on "Registration Documents"

4) There will be three files available. You will need to download the registration form as well as the appropriate language survey form. 

5) You will need a completed registration form, completed survey form, immunizations records, birth certificate, and activity fee to complete the registration process.
